Members are required to provide one of the following documents:

• Utility Bill (Electricity, Water, Telephone, Cable/Internet, or Tax Assessment) OR

• Bank Statement OR

• Rental/Lease Agreement & Utility Bill from the property owner/landlord

 If the utility bill isn’t in your name, please provide:

• An authorization letter from the person whose name appears on the utility bill

• A valid copy of their photo ID

Please note: If you have updated your information within the past 3 months, you will not be required to provide these documents again.
